Where can I find the rules for playing "The Ghost"?

Note that there are multiple ways in which you can "play the ghost" to make it easier or harder as desired.
In working on one's 9-ball game, for example, one might first want to try and defeat the 7-ball ghost before stepping up to the 9-ball ghost. This would be done by racking and breaking 7 balls or using 9 for the breaking and taking a couple of balls off the table after the break shot.
Playing the ball-in-hand ghost (BIH ghost), with no penalties for break scratches will similarly be much easier than playing from the position at which the cue-ball comes to rest after the break.
