I am trying to find out the year and model ID for this "Palmer Custom Cue".

Your cue appears to be a Palmer PM-2 or Palmer PM-3 pool cue from the Palmer PM series, circa the late 1970s.
I can't tell from just these photos, but it's the same cue except the PM-3 has a wrap, whereas the PM-2 cue does not.
The "PM" stood for Pete Margo (a World Champion and Hall of Fame billiard player, and the player who the term "sneaky pete" is named for).
This line was made by Adam cue of Japan according to the design specs of the Palmer Billiard Corporation.
Palmer PM-2 (and PM-3) Pool Cue
This model features a solid Canadian maple butt, equipped with a brass joint with a black fibre trim. A plain, good looking and good hitting cue that has all the components of our higher-priced models.
PM-3 is the same as the PM-2, except that it is wrapped in black dacron.
